Membership fees are billed monthly and are due in advance.
Membership may be canceled at any time with 30 days' written notice.
Annual memberships receive a 10% discount but may also be canceled with 30 days notice. The balance of fees paid will be refunded to the patient.
Membership fees do not include laboratory testing, medications, specialist consultations, imaging studies, hospital services, or other third-party healthcare costs unless specifically stated.
Medical services and treatment recommendations are provided at the sole discretion of the physician based on individual clinical needs.
Participation in the program does not guarantee specific health, weight loss, or treatment outcomes.
Members are responsible for maintaining accurate and up-to-date health information.
